Transitional Living Unit

Ticehurst TLU

The Transitional Living Unit (TLU) is suitable for both adults reaching the end of a transitional placement in a Priory Grange hospital or another rehabilitation service and for those making the transition from adolescent services into adult services.

Our service users

Our service users are typically ready to move on and either:

  • Require long-arm support and supervision to consolidate daily living skills before living fully independently
  • Have not been found a suitable community placement and are in jeopardy of losing momentum and the therapeutic gains they have made
  • Need a chance to practice independence away from, but within reach of, a fully staffed hospital service to help manage attachment anxieties while challenging institutionalization

Service users include those:

  • With long-term mental health problems, who need support with daily life
  • Who may also have social and behavioural diffi culties
  • Who have potential for full rehabilitation into the community long-term
  • Who may be subject to Community Treatment Orders
  • Who may be detained under the Mental Heath Act

Our aim

The unit aims to deliver a fi nal phase of preparation for successful community living to clients who will eventually be managing all aspects of daily life themselves. The service is consultant-led, offering continuity for clients progressing from the Priory Ticehurst Grange as well as other Priory Grange hospitals, and the unit is staffed with a support worker 24 hours a day to provide assistance with activities of daily living.

Our TLU service includes:

  • Intensive rehabilitation programmes overseen by a consultant psychiatrist
  • Planned outcome of transition into a more independent setting
  • Individualised social, personal and educational development programmes
  • Personalised occupational therapy programmes based on the Model of Human Occupation (MOHO) with a Recovery Model influence
  • Support in accessing college and work placements and fully integrating into the local community
  • Monthly multidisciplinary review for first six months and bi-monthly thereafter

Therapeutic programmes

All clients have a personalized occupational therapy programme, with a focus on developing vocational activities and maximising independent living skills. The use of community facilities and integration within the local community will be promoted for clients living on the TLU as part of their rehabilitation programme. This follows the Human Occupation Model with a Recovery Model influence.

Our multidisciplinary team

Clients living on the TLU will benefit from having full access to the multidisciplinary team consisting of:

  • Consultant psychiatrists
  • Clinical services manager
  • Charge nurse and staff nurses
  • Therapists and occupational therapist
  • Support workers and occupational therapy assistants

Facilities

  • Four bedrooms, fully-equipped kitchen, shared lounge and dining area
  • Extensive grounds, art room and sports hall
  • Social and recreational facilities on and off site
  • Rural setting within one mile of Ticehurst village shops
  • Three miles to Wadhurst town centre, three minutes to the bus stop
  • Use of shared hospital vehicle
