We provide tailored and time-lined treatment and intervention programmes for patients through a range of care pathways which enable the patient and team to work towards an agreed projected discharge date:

The assessment-only care pathway

  • A 3 month assessment is followed by a comprehensive needs analysis which is carried out by a full multi disciplinary team. This identifies the most appropriate placement and care pathway for the patient.

The behaviour and mood stabilisation care pathway

  • Individual treatment programmes with clear goals and timelines aimed at reducing and stabilising patient behaviours that are considered to challenge both the patient and others and which inhibit social inclusion
  • Focus on social inclusion, integration and independent living skills
  • Upon completion of the programme, the patient may present with the potential to commence a rehabilitation care pathway

Rehabilitation care pathway

  • Individual treatment programmes with clear goals and timelines for patients to achieve full rehabilitation potential and work towards successful discharge
  • Focus on independent living skills, social inclusion, integration and real work opportunities
  • Locked rehabilitation / Open rehabilitation available

Patient profile for Brain Injury Services:

  • Male or female, 18 years of age or older
  • Diagnosed with a brain injury requiring support, intervention, treatment of behaviour, physical and/or cognitive difficulties
  • Patient with an acquired brain injury and co-existing mental health difficulties
  • Detained under the Mental Health Act (1983) or informal status

We offer a discharge pathway which consists of assessment, intensive rehabilitation/ stabilisation with the option to continue through the discharge pathway to the community houses for continued rehabilitation in a graded exposure programme for sustainable discharge or to an alternative placement. Outreach service and support is also provided following discharge from hospital.
