Background

DC* is a personable and independent man with a diagnosis of depression, type 2 diabetes, and a learning disability. Prior to his admission, he lived with his parents, where he developed strong community ties and a good understanding of his local area. With the support of his mother, DC was able to manage appointments while maintaining his independence, regularly attending college and spending time with friends.

A significant life change

Following the loss of his mother, DC experienced a decline in his mental health, leading to a period of crisis and hospital admission. This was a significant turning point, as he had previously lived in a stable and supportive environment. Moving into Lansdowne Road became an important step in helping him regain stability and rebuild his confidence.

DC’s transition into the service was carefully managed, with input from both his family and professionals involved in his care. His family played a key role in developing his care plan, ensuring that his needs, preferences, and strengths were clearly understood. With guidance, he has developed the confidence to use cleaning products safely and take pride in his environment. Daily life and interests

DC continues to lead an active and fulfilling lifestyle. He attends college twice a week and requires very little support to access this, reflecting the progress he has made. He enjoys activities that support his mental wellbeing, particularly colouring, and has been supported to access resources such as adult colouring books. He also has a strong interest in music and engages well with sessions delivered within the service.

Time in the community remains important to him. He enjoys day trips, visiting farms, exploring new places, and going out for meals. He particularly values spending time with staff during these activities and often expresses his appreciation.

Positive outcomes

Since moving into the service, DC has made meaningful progress. His family report that he is settled, happy, and feels safe in his environment, particularly within his own bedroom, which he views as a secure and comforting space. They have also noticed that he appears calmer and more content in his day-to-day life:
