Background
Mark spent a significant part of his life within forensic services and hospital environments. Over the years, he found it difficult to adapt to life outside of these settings and often experienced anxiety when faced with change. This affected his ability to access the community independently and limited opportunities to pursue the life he wanted for himself.
These challenges also had an impact on family relationships, particularly his relationship with his father.
Presentation and early challenges
When Mark moved to The Gables, he faced a number of challenges that affected his confidence and independence.
- Anxiety when travelling and accessing the community
- Difficulty adapting to change and unfamiliar situations
- Limited confidence using public transport
- Ongoing involvement with community forensic services
- Reduced opportunities for independent living
- Challenges maintaining positive family relationships
A focused and collaborative approach
From the beginning, the focus was on understanding Mark's aspirations, building trusting relationships and providing support that reflected his individual needs and preferences.
The team worked closely with Mark to identify meaningful goals and create opportunities for him to develop confidence, independence and resilience. Support was delivered at a pace that felt comfortable for him, enabling sustainable progress and positive outcomes.
Key elements of support included:
- A planned transition from hospital into the community
- Partnership working with the community forensic team
- Person-centred support tailored to Mark's goals and aspirations
- Gradual exposure and confidence-building approaches
- Support to develop independent living skills
- Encouragement to access community activities and opportunities
- Support to rebuild family relationships
Building independence
One of his most significant achievements has been overcoming anxiety associated with travel. Through a staged approach and gradual exposure work, Mark developed the confidence to use public transport independently. He now travels by bus every day, using local services to access shopping opportunities and activities within the community.
What was once a source of anxiety has become something he enjoys and takes great pride in.
Mark has also made substantial progress in developing his independent living skills. He successfully moved into a self-contained flat, which he maintains with support, and developed the confidence and practical skills to prepare meals independently several times each week.
Daily life and relationships
Mark now accesses his local community regularly, takes part in activities that are important to him and continues to build on the skills he has developed since moving to The Gables.
The team has also supported Mark to rebuild and maintain positive relationships with his family. In particular, he has strengthened his relationship with his father, with family members recognising the significant progress and positive changes he has made.
Looking ahead
The team will continue to provide personalised support that promotes independence, confidence and meaningful participation in community life, ensuring Mark has every opportunity to achieve his goals and maintain the progress he has worked hard to achieve.
A meaningful transition
Mark's journey demonstrates the positive impact that personalised support, partnership working and a strengths-based approach can have on an individual's life.
Through a carefully managed transition, consistent support and a focus on what matters most to him, Mark has achieved outcomes that once seemed out of reach. Today, he enjoys greater independence, stronger family relationships and increased confidence, while continuing to build a meaningful and fulfilling life within the community.
